Witam Was kochani! Jestem w trakcie diagnozowania "czegoś". Mam oczywiście powiększone węzły szyjne (do 10mm w osi długiej, hypoechogeniczne) i pachwinowe (do 10 mm w osi długiej stłuszczałe).
Od dłuższego czasu zajmuję się "studiowaniem" Waszego forum, artykułów medycznych, wypowiedzi lekarzy, statystyk zachorowań na ziarnicę bądź inne chłoniaki, oglądam zdjęcia usg i rtg osób chorych próbując znaleźć odpowiedzi na wszystkie swoje pytania (pytania również osób, które są na tym forum). Powiem Wam jedno (to do wszystkich szukających opowiedzi czy mają nowotwór) jest to niemal pewne, iż nie znajdziecie na internecie niczego co Wam w czymkolwiek pomoże :) Straszne, prawda?
A teraz napiszę pewne streszczenie tego forum dla osób czytających je godzinami i inne dostępne internetowe materiały abyście nie musieli spędzać tyle czasu na czytaniu i rozmowach z lekarzami co ja.
Jakie badania są wykonywane w celu potwierdzenia bądź wykluczenia ziarnicy bądź innego chłoniaka:
1. Badanie krwi Morfologia krwi z rozmazem i OB jest pierwszym badaniem, na które kieruje każdy lekarz. Podstawową rzeczą jest aby zrobić morfologię z rozmazem ręcznym, a nie automatycznym! Rozmaz ręczny jest zdecydowanie bardziej dokładny, jest w stanie wykryć komórki atypowe. Co nam daje w praktyce? Niestety niewiele. Jeżeli w badaniu wyjdą znaczne odchylenia np. obniżona/podwyższona wartość leukocytów (lub poszczególnych białych krwinek - limfocytów, neutrofili itd), obniżone płytki krwi czy obniżone czerwone krwinki - możemy podejrzewać chorobę o charakterze rozrostowym. Podejrzewać! Nie mówię, że już mamy diagnozę! A już napewno nie jest to pewność , że mamy ziarnicę! Z drugiej strony - prawidłowe wyniki krwi nie oznaczają braku choroby - ziarnicy czy innego nowotworu. Szukamy dalej.
OB- Wynik OB jest niemiarodajny. Odczyn Biernackiego - cytuję - jest niespecyficznym markerem stanu zapalnego. Oznacza to, że jego podwyższony poziom wskazuje na istnienie infekcji, ale nie pozwala określić jej lokalizacji ani czynnika sprawczego. Zatem OB jest najczęściej podwyższone z powodu infekcji. Innymi powodami podwyższonego OB są m.in. ciąża, menstruacja u kobiet, niedoleczone zęby i wiele innych. Znacznie podwyższone OB może być również spowodowane nowotworami - mówimy o znacznie podwyższonych wartościach powyżej 20. Teoretycznie tak jest. A praktycznie? OB 20 może nie wskazywać na żadną chorobę - a na stan fizjologiczny, a OB na poziomie 1, nie wyklucza nowotworu! Wiele chorych ma OB na niskim poziomie. Zatem niczego się nie dowiemy. Idziemy dalej.
2. Kolejnym badaniem uzupełniającym jest usg węzłów chłonnych . Tak jak zostało to już określone na tej stronie. Węzły w badaniu usg mogą mieć różną strukturę i wielkość. Według książek medycznych patologicznie powiększony węzeł ma wielkość 1cm-1,5cm średnicy! Więc patologiczne węzły są dość spore. Aczkolwiek wielkość mniejsza niż określona jako patologiczna nie wyklucza procesu nowotworowego. W Usg mogą mieć obraz hiperechogeniczny lub hypoechogeniczny. Mogą posiadać widoczną zatokę tłuszczową, zatartą zatokę tłuszczową lub nie posiadać widocznej zatoki tłuszczowej. Węzły hiperechogeniczne są z reguły węzłami z widoczną zatoką tłuszczową co oznacza, że są nieaktywne, stłuszczałe. Takie węzły są niemal napewno nienowotworowe, niepodejrzane, stare węzły po przebytym zapaleniu. Węzły hypoechogeniczne są węzłami aktywnymi, mogą być zarówno aktywne odczynono jak i aktywne nowotworowo. Węzły mogą mieć również prawidłowy przepływ w badaniu dopplerowskim lub nieprawidłowy. Mogą również mieć prawidłową echostrukturę lub nieprawidłową echostrukturę - i tu zaczyna się mętlik co oznacza echostruktura? Prawdopodobnie właśnie nawiązuje do hiper bądź hypoechogeniczności. Jeżęli mamy węzeł hypoechogeniczny (ciemny w badaniu usg) możemy mieć alergię, zapalenie, zakażenie itp lub nowotwór. Niestety na tym moc usg się kończy, bardzo rzadko usg daje jakąś konkretną odpowiedź, zwykle nie daje żadnej. W tym miejscu chcę zaznaczyć, iż posiadanie węzłów odczynowych jest dużo bardziej prawdopodobne niż węzłów nowotorowych. Zachorowalność na chłoniaka w Polsce to 6000 nowych zachorowań rocznie. Szukamy więc dalej.
3. Prześwietlenie klatki piersiowej RTG. RTG klatki piersiowej jest badaniem bardzo przydatnym w badaniach okresowych, pozwala na wykrycie wielu zmian w obrębie klatki piersiowej, ale niestety jak wskazuje na to praktyka, nie daje żadnej pewności. Jest to badanie, w którym nie trudno pominąć istotne zmiany, w zależności od położenia węzłów chłonnych czy guza, radiolog może ich nie zauważyć pomiędzy innymi narządami, sercem, płucami czy żebrami. Badanie RTG może nie wykryć nawet guza wielkości 5cm. Zatem czyste/prawidłowe rtg płuc nie wyklucza ani ziarnicy ani nawet nowotworów płuc. Jeżeli jednak RTG wykaże powiększone węzły w śródpiersiu to mamy na 100% do czynienia z jakąś poważniejszą chorobą - może to być m.in. chłoniak, ziarnica, sarkoidoza.
4. Tomografia komputerowa płuc/Rezonans płuc - to badanie bardzo dokładne, nie pozwoli na pominięcie nawet najdrobniejszych zmian w płucach. Ale nie jest to badanie pierwszego rzutu... niestety, tomografia komputerowa nie jest obojętna dla organizmu, ale daje dużo odpowiedzi. Rezonans płuc jest znacznie bezpieczniejszym badaniem, ma tylko jedną wadę - cenę- 600zł. Lekarze napewno Was na to nie skierują, a samemu takie pieniądze wyłożyć jest ciężko. Czysta TK i RM nie oznaczają braku ziarnicy, powiększone węzły mogą być tylko np. na szyi. Szukamy zatem dalej.
5. Badanie PET - Pozytronowa Tomografia Emisyjna - świetne badanie na wykrycie każdego raka! Widzi każdą aktywną zmianę nowotworową. Niestety jej koszt jest ogromny - ponad 4 500zł. Najlepiej by było od tego zaczynać ;) Niestey- takiej możliwości brak - badanie PET wykorzystywane jest głównie do monitorowania osób, które już chorują bądź chorowały na nowotwór.
6. Biopsja cienkoigłowa - biopsja aspiracyjna cienkoigłowa (BAC) - jest rodzajem badania cytopatologicznego i polega na pobraniu próbki komórek za pomocą cienkiej igły wprowadzonej do tkanki i przyłożeniu podciśnienia (pociągnięcie tłoka strzykawki podłączonej do igły). To badanie nie wnosi wiele. Biopsja może wykazać, że pobrany materiał jest zmieniony odczynowo lub zawiera komórki nowotworowe. Metoda ta w praktyce jest bardzo zawodna, poczas pobrania można pominąć komórki zmienione nowotworowo z węzła i pobrać jedynie te prawidłowe. Nasze szczęście z pozytywnego wyniku badania może okazać się zatem złudne. Jeżeli BAC wykaże, że węzeł zmieniony jest odczynono, nie wyklucza to ziarnicy! Jeżeli wykazuje zmiany nowotworowe wtedy zawsze nazazuje się wycięcie węzła w całości dla określenia typu nowotworu.
7. Biopsja węzłów chłonnych - cytuję:Węzły chłonne do diagnostyki pod mikroskopem pobiera się w całości, razem z ich torebką. Badanie to pozwala na dokładne obejrzenie w całości węzła pod mokroskopem, nie ma możliwości pominięcia komórek nowotworowych w danym węźle (nie licząc braku doświadczenia lekarza ;) ). Ale! Niestety i tu nie można być pewnym na 100% niczego. W ciele człowieka jest wiele węzłów chłonnych. Na szyi możemy mieć zarówno węzły powiększone w skutek zwykłego przeziębienia jak i w skutek rozrostu nowotworowego. Co to oznacza? A oznacza to niestety, że do badania może zostać pobrany zdrowy odczynowy węzeł gdy obok jest ten nowotworowy... Kolejne badanie, które w 100% nie wykluczy choroby, ale jest na to spora szansa gdyż ze sporym prawdopodobieństwem jeżeli jeden węzeł jest nie zajęty nowotworowo to pozostałe podobnie.
I tu pomysły na dalsze badania się kończą. Reasumując.
Badania, które niczego pewnego nie wnoszą: Morfologia, OB, RTG klatki piersiowej, biopsja cienkoigłowa.
Badania, które mogą dać odpowiedź na 70% - tomografia komputerowa, pobranie całego węzła do badania.
Badanie, które daje odpowiedź na 100% - PET.
A co z objawami ogólnymi?
1. Poty nocne - mogą być spowodowane zwykłym przeziębieniem, grypą, nerwicą.
2. Gorączka - prawidłowa temperatura człowieka waha się od 36 do 37.3 stopni. W zależności od pory dnia, dnia cyklu u kobiet, stresu, wysiłku fizycznego może się ona zmieniać. Temperatura do 37.5 to nie gorączka, a stan podgorączkowy. U niektórych temperatura 37 to normalna temperatura ciała. Nie popadajmy więc w panikę gdy mierząc temperaturę okaże się, że mamy około 37 :)
3. Świąd skóry - może być także spowodowany np. alergią, pasożytami, nerwicą itp.
4. Utrata masy ciała- zaznaczam tu, że chodzi o utratę masy ciała w krótkim czasie bez zmiany diety. Może być spowodowana również pasożytami. Ale objaw ten zwłaszcza jeżeli nie uprawiamy sportu, odżywiamy się normalnie wymaga pełnej diagnostyki.
5. Poalkoholowe bóle węzłów chłonnych - Ten objaw na 90% oznacza ziarnicę.. zostawiłam 10% niepewności , jako ,że mogą być inne powody, jeszcze nie opisane nigdzie. Jeżeli macie takie objawy to zdecydowanie wymaga to dalszej diagnostyki.
6. Ogólne osłabienie- tutaj powodów może być milion.. nie jest to objaw zarezerwowany dla nowotworów...
7. Duszności - mogą być spowodowane nerwicą, astmą, alergią, zapaleniem płuc..
Objawy, które zmusząją do obowiązkowego natychmiastowego zgłoszenia się do onkologa/hematologa:
1. Powiększone hypoechogeniczne węzły o średnicy powyżej 1cm.
2. Gorączka powyżej 37.5 utrzymująca się powyżej dwóch tygodni bez innych objawów.
3. Zmniejszenie masy ciała bez diety, uprawiaia sportów itp.
4. Bardzo uporczywy świąd skóry, przy niezmienionym wyglądzie skóry, powodujący duży dyskomfort.
5. Zlewne poty nocne - mowa o potach, które powodują, że musimy zmienić ubranie.
6. Znaczne odchylenia w badaniach laboratoryjnych -morfologia i OB - bez objawów przeziębienia i grypy. |

Dopóki nie "wywali" Wam wielkiego węzła w widocznym miejscu nie ma co liczyć na szybkie postawienie diagnozy :(
Ja jestem na etapie czekania, jeszcze nie wiem na co :) Choć mam dużą wiedzę (tak mi się przyjamniej widaje) to nie wiem nic. Na wszystko potrzeba skierowań od lekarzy - a do tego są niechętni więc badania, których mieć nie będę to:
pobranie węzła do badania, TK/CT , PET.
Lekarze, których odwiedziłam: onkolog x1, internista x3, hematolog x2, larynolog x3, endokrynolog x 3, usg węzłów chłonnych x5, morfologia i OB x4, RTG - dziś odbieram wynik. Czas podjęcia powyższych działań - grudzień 2011.

U mnie jest tak:
Węzły szyjne są powiększone hypoechogenne o niejednorodnej echogeniczności, mam skierowanie na kolejne USG tym razem podobno dokładniejsze, do tego dochodzi skierownie na USG brzucha (pobolewa mnie brzuch, mam jakas dziwna asymetrię wyczuwam i widze pod reka "zgrubienie" o owlanym kształcie nie bolace)oraz USG tarczycy.
Obnizyl mi sie glos!!!
Morfologia generalnie ok tylko Neut lekko podwyzszone a LYMPH obnizone
OB podwyższone 21
CRP 1.57 wiec ok
Objawy
Powiekszone wezly, swiad skóry ale nie umiejscowiony (czasami mocniej swedzi szyja)swiad jest dosc uporczywy, bez zmian skornych , "rwanie węzłów szyjnych", wzmozona potliwość, osłabnięcie, senność, pobolewanie gardla i ucha (brak zapalenia gardla i ucha) bole w pleców umiejscowione (dokladnie na odcinku piersiowym - po nocy ale moze tylko źle spie)
RTG wyszło prawidłowo
Czytałam o poalkoholowym bólu węzłów, ale generalnie nie zaobserwowałam prędzej ból migdałów ale może się nie znam i coś mylę.
W każdym razie jestem w trakcie badań a wasze forum pomaga mi poradzić sobie okropnym uczuciem jakie towarzyszy mi kiedy musze opowiedziec lekarzowi co mi jest.
Chodzi o to ze czuje sie jak hipochondryk a kiedy dostrzegam ich wilkie widrujace oczy mam ochote wyjsc.
Pierwszy raz badam się tak dokładnie od 5 lat (nie liczac odwiedzin u gin) namówił mnie od tego małżonek zaniepokojony moim ciągłym osłabieniem i tak naprawde z czym innym poszłam do lakarza a co innego wyszło!
I jeszcze jeden szczegół... w rodzinie miałam przypadki ziarnicy jak i samego raka w różnej postaci.
Nie panikuję aczkolwiek ciocia mocno mnie nastraszyła.
Ona byla diagnozowana ok 8 miesiecy na pczątku brano ja za depresantke poniewaz badania wychodzily jej ok a ona wciaz źle się czula dopiro po dokladniejszych badaniach i niestety w pózniejszym czasie wyszły złe wyniki.
Niestety mój kochany dziadek nie miał aż tyle szczęścia.

Ale nie jest to badanie pierwszego rzutu... niestety, tomografia komputerowa nie jest obojętna dla organizmu, ale daje dużo odpowiedzi. Rezonans płuc jest znacznie bezpieczniejszym badaniem, ma tylko jedną wadę - cenę- 600zł. Lekarze napewno Was na to nie skierują, a samemu takie pieniądze wyłożyć jest ciężko. Czysta TK i RM nie oznaczają braku ziarnicy, powiększone węzły mogą być tylko np. na szyi. Szukamy zatem dalej.

> 5. Badanie PET - Pozytronowa Tomografia Emisyjna - świetne badanie na wykrycie każdego raka! Widzi każdą aktywną zmianę nowotworową. Niestety jej koszt jest ogromny - ponad 4 500zł. Najlepiej by było od tego zaczynać ;) Niestey- takiej możliwości brak - badanie PET wykorzystywane jest głównie do monitorowania osób, które już chorują bądź chorowały na nowotwór.

> 6. Biopsja cienkoigłowa - biopsja aspiracyjna cienkoigłowa (BAC) - jest rodzajem badania cytopatologicznego i polega na pobraniu próbki komórek za pomocą cienkiej igły wprowadzonej do tkanki i przyłożeniu podciśnienia (pociągnięcie tłoka strzykawki podłączonej do igły). To badanie nie wnosi wiele. Biopsja może wykazać, że pobrany materiał jest zmieniony odczynowo lub zawiera komórki nowotworowe. Metoda ta w praktyce jest bardzo zawodna, poczas pobrania można pominąć komórki zmienione nowotworowo z węzła i pobrać jedynie te prawidłowe. Nasze szczęście z pozytywnego wyniku badania może okazać się zatem złudne. Jeżeli BAC wykaże, że węzeł zmieniony jest odczynono, nie wyklucza to ziarnicy! Jeżeli wykazuje zmiany nowotworowe wtedy zawsze nazazuje się wycięcie węzła w całości dla określenia typu nowotworu.

> 7. Biopsja węzłów chłonnych - cytuję:Węzły chłonne do diagnostyki pod mikroskopem pobiera się w całości, razem z ich torebką. Badanie to pozwala na dokładne obejrzenie w całości węzła pod mokroskopem, nie ma możliwości pominięcia komórek nowotworowych w danym węźle (nie licząc braku doświadczenia lekarza ;) ). Ale! Niestety i tu nie można być pewnym na 100% niczego. W ciele człowieka jest wiele węzłów chłonnych. Na szyi możemy mieć zarówno węzły powiększone w skutek zwykłego przeziębienia jak i w skutek rozrostu nowotworowego. Co to oznacza? A oznacza to niestety, że do badania może zostać pobrany zdrowy odczynowy węzeł gdy obok jest ten nowotworowy... Kolejne badanie, które w 100% nie wykluczy choroby, ale jest na to spora szansa gdyż ze sporym prawdopodobieństwem jeżeli jeden węzeł jest nie zajęty nowotworowo to pozostałe podobnie.

> I tu pomysły na dalsze badania się kończą. Reasumując.
> Badania, które niczego pewnego nie wnoszą: Morfologia, OB, RTG klatki piersiowej, biopsja cienkoigłowa.
> Badania, które mogą dać odpowiedź na 70% - tomografia komputerowa, pobranie całego węzła do badania.
> Badanie, które daje odpowiedź na 100% - PET.

> A co z objawami ogólnymi?
> 1. Poty nocne - mogą być spowodowane zwykłym przeziębieniem, grypą, nerwicą.
> 2. Gorączka - prawidłowa temperatura człowieka waha się od 36 do 37.3 stopni. W zależności od pory dnia, dnia cyklu u kobiet, stresu, wysiłku fizycznego może się ona zmieniać. Temperatura do 37.5 to nie gorączka, a stan podgorączkowy. U niektórych temperatura 37 to normalna temperatura ciała. Nie popadajmy więc w panikę gdy mierząc temperaturę okaże się, że mamy około 37 :)
> 3. Świąd skóry - może być także spowodowany np. alergią, pasożytami, nerwicą itp.
> 4. Utrata masy ciała- zaznaczam tu, że chodzi o utratę masy ciała w krótkim czasie bez zmiany diety. Może być spowodowana również pasożytami. Ale objaw ten zwłaszcza jeżeli nie uprawiamy sportu, odżywiamy się normalnie wymaga pełnej diagnostyki.
> 5. Poalkoholowe bóle węzłów chłonnych - Ten objaw na 90% oznacza ziarnicę.. zostawiłam 10% niepewności , jako ,że mogą być inne powody, jeszcze nie opisane nigdzie. Jeżeli macie takie objawy to zdecydowanie wymaga to dalszej diagnostyki.
> 6. Ogólne osłabienie- tutaj powodów może być milion.. nie jest to objaw zarezerwowany dla nowotworów...
> 7. Duszności - mogą być spowodowane nerwicą, astmą, alergią, zapaleniem płuc..

Na PET niestety mało kto może liczyć.... Nikt nie pomysli, że oszczędziło by to po drodze wiele badań i wizyt u tzw. "lekarzy", którzy traktują człowieka jak lenia, wyudzacza renty, zwolnienia, zasiłku czy czegos tam jeszcze. Ja przez dwa lata nie robiłam żadnych badań od lekarza POZ, nawet morfologii a i tak mi dał skierowania (dwie siostry po nowotworze i tata) a on się pyta do czego mi morfologia potrzebna. Musiałam zrobić prywatnie, dał mi tylko na usg brzucha. Teraz czekam na wynik trepanobiopsji.

Witaj. Myśle, że skoro walczysz o diagnozę od 2011 roku to chyba nie ma powodów do obaw, że to nowotwór. Jednak nie jestem lekarzem i z tego co mi mówił onkolog, ludzie chodzą do niego czasem 3 lata i nie mogą się dowiedzieć co im dolega. Podobno tacy pacjenci lawirują pomiędzy nowotworem a zapaleniem organizmu, infekcją itp. Nie do końca wiadomo co się w organizmie dzieje i na jaką szalę się wszystko przechyli. Ja w porównaniu do Ciebie dopiero raczkuję w oczekiwaniu na diagnozę bo czekam od maja 2013:) miałem 3 razy morfologię. Za każdym razem inne wyniki. 3 razy USG, ale chyba radiolodzy u mnie w mieście nie bardzo się orientują w sprawach limfadenopatii bo nie potrafią niczego powiedzieć po badaniu. Absolutnie niczego. Dwa razy byłem u laryngologa, którzy stwierdził, że wszystko jest ok. Kilka razy u hematologa, a nawet lekarza od chorób naczyń i chorób zakaźnych. Do tego gastroenterolog i hepatolog. Niebawem będę miał w kolekcji cały pakiet poradni. Miałem wycinany jeden węzeł a teraz czekają mnie dwie biopsje. Nie ukrywam, że żyję w stresie bo nie wiem co sie dzieje. Nie mogę podjąć każdej pracy bo wyniki są nie do końca ok i lekarz ich nie podpisze. Ciekaw jestem Twoich wyników. Daj znać co i jak

w moim przypadku - odkąd wykryto raka u dwójki przyjaciół w bardzo krótkim odstępie czasu - pojawiła się, niestety, hipochondria.
Latem 2012 roku wykryłem i siebie guzek pod żuchwą, pierwsze USG nie pokazało niczego. przypomniałem sobie o nim jesienią - kolejne USG pokazało już coś o wymiarach 17x14 mm. Dalej USG kontrolne i biopsja cienkoigłowa - wynik: odczynowy węzeł chłonny, komórek podejrzanych nie znaleziono. Lekarz prowadzący poklepał po plecach i uspokoił (morfologia była ok), lekarz przeprowadzający biopsję jeszcze widząc obraz USG także stwierdził, że nie wygląda to podejrzanie.
Teraz, rok po tym badaniu, stan zapalny gardła (zdarza się z racji krzywej przegrody nosowej) i wspomniany węzeł boleśnie się powiększył. Trwał tak trzy dni, teraz znowu zmalał i właściwie nie boli. Oczywiście - wspomnienia (i panika) wracają... morfologia wykonana w lipcu była doskonała (odrobinę za niskie MCV, reszta idealna), a mnie znowu kusi, by pójść prywatnie na USG i nakręcać tę spiralę od nowa.
Dodam, że wspomniany węzeł to chyba jedyny objaw, gorączki/pocenia się/spadku masy ciała i siły brak (siłownia regularnie od trzech lat). Czy mimo to powinienem się niepokoić? Gdyby była to ziarnica lub cokolwiek groźnego - na pewno w ciągu ponad roku wystąpiłyby inne objawy, prawda?
